Notes on submitting your consents

  • Watch the time limit; send all of the consent forms to us within 20 days of when the company name was reserved. 

    Note | Consent forms need to be received and accepted by 5pm (NZST) on the last working day prior to your application expiring. 

    We will send you a short series of reminder emails but if this third and final step of the application process is not completed within the timeframe, all information will be deleted from the system and any fees paid will not be refunded.  You would need to begin the entire process again and pay the fees again.
  • Do not include a cover sheet or send any other documents for registration to this fax number.  It is a dedicated line for receiving director and shareholder consent forms.  The consent forms you send to this number are imaged as they are received, no paper document is generated.
  • Each consent form must be faxed as a separate A4-sized page, but you can send more than one page at a time.
  • Keep the original consent forms as part of the company's records.

 

Note | There might be multiple shareholder consent forms for a single shareholder.  Usuallly this is when they hold shares in several share allocations (for example, in their own right and as a joint shareholding with others).  The shareholder is required to sign and return all forms.
